i think it's the facial expression that makes what would otherwise just be another vaguely suggestively kinky screencap into something truly brilliant. possibly aiba is working on a plan to take over the universe and every aspect of his expression is studied. or possibly he's really thoughtful and distracted, with his mouth full of riceball. (i have to admit, i vote for the former, though. maybe when jun aims the camera at him a few minutes later, instead of at the grandparents, he's internally cackling with smug glee, because everything is going according to plan. maybe that's why sho is so dubious - he suspects what aiba is up to!)
