A family connection is involved with a USA-wide "Community Health Needs Questionnaire". It's online here and they want any and all US-resident participants. I hope they're going to get a sufficiently broad and random sample via viral 'net marketing - it seems a little dubious, so please help them out or pass on the link, if you can.
