Maybe in that universe, it's not like, contiguous - they still have other conversations and Dean still sleeps and pees and Cas still does his job - but just like, every time they meet, Dean constantly stuffs Things That Suck into all the conversational cracks. Cas: Only you can get through to your brother because God just can't think of any other way, Dean. Dean: Sammy's my responsibility! And what about gas stations with only one bathroom, huh? And dude, lose the trench. John Constantine is not a cool look for you.
